Amy (Babcock) Fowlkes - MS, MPT, Clinic Director

Amy Fowlkes Amy received her Master of Science degree in Exercise Physiology with an emphasis in nutrition at Iowa State University in 1993 and her Masters in Physical Therapy at St. Ambrose University in 1996. In addition she has completed three long term courses in Orthopedic Manual Therapy (The Australian Approach). Amy has taken over 500 hours of post graduate PT training, 350 of which were in manual therapy. She worked at the St. Anthony Spine Center in Oklahoma City, OK. and went on to work as the Spine Coordinator at Accelerated Rehab in Ames, IA. She also worked at Oklahoma State University and had the opportunity to work with athletes in their rehabilitation process. She is a member of AAOMPT.

Amy believes in precision with exercise and specific manual techniques, as well as education on posture and alignment to facilitate recovery. Amy’s professional interests include Orthopedic Manual Therapy, Spine and Sports Rehabilitation, Biomechanics/Orthotics (foot and ankle), Bike Fit Analysis, Therapeutic Yoga and post-operative care. She is now a Certified Anusara Inspired Yoga Instructor and teaches weekly classes at the clinic. She has mentored PT staff at Mountain View PT in manual therapy since 2002.

Amy competed as a triathlete for 8 years, then competed at the national level as a category 1 competitive road cyclist for another 8 years. During her undergraduate years, she was a competitive collegiate track and cross country runner. She now spends her free time with her husband, son and daughter, riding locally, practicing yoga, and swimming.

Jeremy Chan - DPT, ATC

 Jeremy Chan Jeremy graduated from UCLA with an undergraduate degree in Psychobiology and completed UCLA’s Athletic Training Internship Program. After graduating, he stayed one year to work with the UCLA baseball team, culminating in a trip to the NCAA College World Series in 1997. He attended the University of Southern California and received his Doctorate of Physical Therapy degree in 2002. Jeremy’s professional interests include the use of manual therapy, muscle energy techniques, throwing and overhead sports injuries, biomechanics, orthopedics, foot and ankle rehabilitation, and pediatrics. He is currently a member of the Orthopedic, Sports, and Pediatric sections of the American Physical Therapy Association and is a member of the National Athletic Trainers’ Association. Outside of work, Jeremy sponsors a high school youth group, and enjoys participating in baseball and basketball.

Brenda Lee - MSPT

Helen LiuBrenda graduated with a Bachelor of Science degree from Brigham Young University, Utah in Physical Education with an emphasis on Pre-Physical Therapy. She received her Master of Science in Physical Therapy from the University of California, San Francisco in 2003. Her professional interests include an exercise based approach to therapy, manual therapy, biomechanics, foot and ankle rehabilitation, and general orthopedics. She also has basic German language skills. Outside of work, Brenda is very involved in various church activities and participates in all sports, especially basketball.

 

Chaula Vakil - MPT

Chaula Vakil Chaula received her Masters in Physical Therapy from University of Maryland, Baltimore. She has also completed a year-long Australian approach manual therapy course for spine and extremities. She received the Laura G. Levine-Mendell award which recognizes a graduating student who demonstrates caring and compassion that makes an outstanding clinician. Chaula’s professional interests include orthopedic manual therapy and ergonomics. She is also proficient in Hindi and Gujarati. In her free time, she enjoys swimming, doing yoga, and increasing the awareness of essentials in proper ergonomics in daily living to prevent injuries.


 

Chi Tso - PT

Chi Tso Chi received her Bachelors of Physical Therapy degree, with honors, from Queen’s University, Canada in 1991. Since graduation, she has taken various continued education courses with interest in orthopedic manual therapy, McKenzie technique, Mulligan concept, Sahrmann and acupuncture. She has worked 7 years in Canada in both orthopaedics and neurological setting before moving to California. Her professional interests include orthopedic manual therapy, neurological and acupuncture. She is also fluent in Cantonese. Away from work, she enjoys an active lifestyle with activities such as hiking, skiing and cycling.


 

Palak Shah - PT

Palak Shah Palak received her Bachelors of Science in Physiotherapy from Manipal Academy of Higher Education from Kamataka, India in 2006. Palak’s professional interests include orthopedic manual therapy, functional training, muscle energy techniques, and knee injuries. She is also fluent in Hindi, Marathi, and Gujarathi. Outside of work, she enjoys hiking, playing table tennis, bollywood dancing, practicing Pranayam, which is Indian breathing and meditation exercises, as well as spending quality time with her husband and son.


 

Meenal Mujumdar - PT

Meenal Mujumdar Meenal graduated with her Bachelor of Physical Therapy from Maharashta University of Health Sciences in India in 2003. She has her Certificate of Achievement in Pregnancy and Postpartum Physical Therapy – OB (CAPP-OB) level I and level II. She also has her Certificate of attendance in Pelvic Postpartum Physical Therapy (CAPP-Pelvic) level I and level II. Her professional interest is that of Women’s Health, as well as orthopedic manual therapy. She is also able to speak Marathi, Hindi, Panjabi and basic Spanish. Meenal’s personal interests include hiking, parasailing, Indian classical music and spending time with her family.


 

Sam Castillo - PTA

Sam Castillo Sam has his Associate of Arts in Physical Therapy Assistant. His professional interests include manual therapy, orthopedics, and an exercise-based approach to therapy. In Sam’s spare time, he loves home improvement projects, church activities, and all sports.


 

Jerry hung - PT

Jerry Hung Jerry received his Doctorate of Physical Therapy degree from Daemen College in 2009. His professional interests include manual therapy, exercise-based approach to therapy, and muscle energy techniques. He is also fluent in Mandarin and Taiwanese. Outside of work, he enjoys playing basketball and golfing.


 

Manisha Shahani - PT

Manisha Shahani Manisha received her Bachelor of Science degree from the University of Michigan followed by a Doctorate of Physical Therapy degree from Northwestern University in 2003. Her professional interests include orthopedics, manual therapy, and vestibular therapy. She is also proficient in both Spanish and Gujarati.
Outside of work, she enjoys running, cooking, traveling, reading, and spending time with her husband and daughter.


 

Karen Van Valkenburg - OT

Karen Van Valkenburg A native of Maine, Karen received a Bachelor of Science degree in Occupational Therapy from the University of New Hampshire. She has specialized in hand therapy for over 12 years and became a Certified Hand Therapist in 2004. She has worked in hand therapy on both coasts and in the mid-west; employed by major sports rehabilitation corporations, hospitals and private practices. She enjoys working with a diverse caseload – fractures and post-surgical injuries are her favorite. Karen is an outdoor enthusiast who enjoys skiing, golfing, and tennis. When not working, she loves traveling and spending time with her husband and daughter.
